Diabetes Self-Care Programmes

Learn more about the comprehensive programmes designed to help individuals with Type 1 and Type 2 diabetes lower blood glucose levels, reduce the need for medication and achieve an overall feeling of wellbeing.

Diabetes Self-management Education and Support

 

Provider:
Fountain Health

Participation Criteria:

  • Diagnosed Type 1 or Type 2 diabetes
  • Children and adults
  • No exclusions for existing diabetes-related complications

Programme Duration:
12 months

Programme Structure:

  • Initial consultation and follow up with Endocrinologist (6 visits)
  • Group sessions (10 hourse)
  • One-to one sessions with RD and RN (13 visits), with point-of-care blood glucose testing
  • Podiatry care (4 visits)
  • Retinal imaging (1)

Care Team:

  • Endocrinologist
  • Nurse/ Certified Diabetes Educator
  • Registered Dietitian
  • Podiatrist
  • Ophthalmologist/ Retinal Screen Technician

Diabetes Reversal Programme

 

 

Provider:
Premier Health & Wellness Center in partnership with the Bermuda Diabetes Association

Participation Criteria:

  • Diagnosed Type 2 diabetes
  • Adults older than 18 years
  • Existing diabetes-related complications (e.g. eye, nerve, kidney damage, etc.) could be excluded upon initial assessment

Programme Duration:
12 months

Programme Structure:

  • Initial consultation and follow up with Endocrinologist
  • Follow up with Endocrinologist every two months and Final assessment (Total 5 -6 visits)
  • Six week course, group setting
  • Course follow up every two months, group setting (Total 4-5 visits)
  • 12 month program

Care Team:

  • Endocrinologist
  • General Practitioner
  • Registered Dietitian/ Certified Diabetes Educator

Diabetes Self-management Education (DSME)

 

Provider:
Bermuda Hospitals Board D.R.E.A.M. Center

Participation Criteria:

  • Diagnosed Type 1 or Type 2

Programme Duration:
12-hour course

Programme Structure:

  • One-on-one appointments in interactive classroom setting

Care Team:

  • Endocrinologist
  • Registered Dietitian

Services not covered include:


Lab tests (covered in Diagnostic Services benefit), food or food/vitamin supplements, educational materials, use of exercise equipment, individual exercise sessions or gym memberships, diagnostic or follow up testing not deemed to be medically necessary, including genomics; experimental or investigational treatments for weight reduction. This includes, but is not limited to, acupuncture or laser therapy for weight loss, body plethysmography, dual-energy x-ray body composition, gastric electrical stimulation, HCG or vitamin injections for weight loss, indirect calorimetry, whole body calorimetry and whole body bioimpedence analysis.
